Explicit Consent Declaration

VALIDE HUMANITARIAN AID ASSOCIATION — DECLARATION OF EXPLICIT CONSENT UNDER THE LAW ON THE PROTECTION OF PERSONAL DATA. Within the framework of the General Privacy Notice under the Law on the Protection of Personal Data and the Association’s Regulation on the Protection and Processing of Personal Data, presented to your information by Valide Humanitarian Aid Association (the “Association”) in accordance with the relevant provisions of Law No. 6698 on the Protection of Personal Data (the “PDP Law”),

I accept and declare that my identity information, address information, contact information and other personal data directly or indirectly related to the following purposes may be processed by the Association in its capacity as data controller, or by data processors authorised by it after ensuring the necessary security measures are taken: providing the products and services offered to the Association’s donors under the best possible conditions; supplying those products or services reliably and without interruption; maximising donor satisfaction; making payments; carrying out various transactions relating to those services; conducting and developing operations; performing promotional, marketing, advertising and campaign activities for those or other products and services; informing donors of announcements, campaigns and other services; and fulfilling the requirements of agreements concluded with donors. I further accept that such data may be obtained, recorded, securely stored in physical or electronic form for a period appropriate to the purpose of processing, retained, modified, reorganised, disclosed and transferred in accordance with legislation, assigned, classified and processed, or its use restricted, in line with the general principles set out in Article 4 of the PDP Law — above all the principle of retention only for the period required by legislation or necessary for the purpose of processing — and that I have been informed by the Association regarding the matters stated above and that my explicit consent exists within the framework of the PDP Law.

I accept and declare that, provided it remains bound by the purposes stated above, my personal data may be transferred by the Association to its employees and officers, to its group entities (the Association and/or its business partners), to public institutions and organisations legally authorised to receive it, and — within the framework of legal obligations and statutory limitations, in order to carry out its activities — to independent audit firms, survey companies, and to the business partners and service providers from which the Association receives services or with which it works in order to perform the services and/or activities to be provided to me; and that I give my explicit consent in this respect.

I further acknowledge that, pursuant to Article 11 of the PDP Law and the relevant legislation, I have the right to apply to the Association and, in respect of myself: to learn whether my personal data is processed; to request information if it has been processed; to learn the purpose of processing and whether the data is used in accordance with that purpose; to know the third parties in Turkey or abroad to whom the data has been transferred; to request rectification where the data has been processed incompletely or inaccurately; to request erasure or destruction of my personal data where the grounds requiring its processing cease to exist; to request that such rectification and erasure be notified to third parties to whom the data has been transferred; to object to an outcome adverse to me arising from the analysis of processed data exclusively by automated systems; and to claim compensation for damage suffered as a result of unlawful processing of my personal data. I acknowledge that, in order to exercise these rights, I may submit my request — containing the information necessary to establish my identity together with explanations concerning the right I wish to exercise — by completing the form available at www.valide.org.tr and delivering a signed copy of the form in person, together with identity documents, to the Association Directorate at “Fevzi Çakmak Mah. 1104 Sok. No: 42/A, Esenler, İstanbul”, or by sending it through a notary public or by the other methods specified in the PDP Law.

I also accept and declare that the personal data I have shared with the Association is accurate and up to date, and that I shall notify the Association of any changes to this information.

I accept that I give my explicit consent to the processing of my personal data, including special categories of personal data as defined in the PDP Law, to its use and disclosure limited to the purpose of processing within the relevant process, and to its retention for the required period; that the necessary information has been provided to me in this respect; and that I have read and understood this text, the Association’s Regulation on the Protection and Processing of Personal Data and the Privacy Notice.
